We understand that choosing to surrender a pet is often a very difficult and emotional decision. This application is designed to help us learn as much as possible about your pet’s history, health, behavior, and daily needs so we can make the most informed decision for their well-being. Please complete every section fully and honestly, as all information requested is required unless otherwise noted. Submission of this application does not guarantee that your pet will be accepted into the shelter. Intake decisions are based on space, available resources, and our ability to safely and appropriately care for each animal. Providing complete and accurate information gives us the best chance to properly evaluate your pet’s needs and determine whether we are able to help.
